Fill out our form online

Fill out our "Interested in Our Services" form

Our Admin team will contact you to verify your information and answer any questions you may have

While verifying your information with us, you will be notified that we have a call-back list for your area and be asked if you would like to be added to this list

We contact you

You will be contacted when you are at the top of our callback list and we have an opening to check:

You still need ABA Services​

Your availability will work with the schedule

Any other questions the BCBA has

Ensure we are a good match

You will be asked to fill out our New Client Form to make sure that we are a good fit for you and your family and that you are a good fit for us

If everything is good with the form, you begin the onboarding process

Onboarding

STEP 1 - Making your Profile on Central Reach (CR)

HIPPA-approved platform we use for data, notes, forms, documents, reports, and assessments.

STEP 2 - Reading, filling out and signing forms and uploading documents

Diagnosis Evaluation, IEP, Insurance Cards
-

STEP 3 - We call your Insurance and fill out the Eligibility form. Then, you look this form over and sign the patient responsibility form
