Fill out our form online

  • Fill out our "Interested in Our Services" form

  • Our Admin team will contact you to verify your information and answer any questions you may have

  • While verifying your information with us, you will be notified that we have a call-back list for your area and be asked if you would like to be added to this list

We contact you

  • You will be contacted when you are at the top of our callback list and we have an opening to check:

  1. You still need ABA Services​

  2. Your availability will work with the schedule

  3. Any other questions the BCBA has

Ensure we are a good match

  • You will be asked to fill out our New Client Form to make sure that we are a good fit for you and your family and that you are a good fit for us

  • If everything is good with the form, you begin the onboarding process

Onboarding

  • STEP 1 - Making your Profile on Central Reach (CR)

    • HIPPA-approved platform we use for data, notes, forms, documents, reports, and assessments. 

  • STEP 2 - Reading, filling out and signing forms and uploading documents

    • Diagnosis Evaluation, IEP, Insurance Cards

  • STEP 3 - We call your Insurance and fill out the Eligibility form. Then, you look this form over and sign the patient responsibility form
